Become a Substitute
Is a team needing you to be a substitute? Follow these steps to register.
STEP 1: Fill out the following form and inform your team captain.
Submit the following form.
STEP 2: Team Captain/Representative informs the board of the new substitute and asks them to check on status of the waiver **MUST BE DONE PRIOR TO ANY GAMES BEING PLAYED WITH THE SUBSTITUTE PLAYER, OR RISK FORFEITURE**
Team captains/representatives must reach out to the GPLVL Board prior to game start.

CAPTAINS WILL NEED TO REQUEST ACCESS, ACCESS WILL ONLY BE GRANTED TO THE EMAIL ADDRESS OF THE CAPTAIN AND CO-CAPTAIN OF THE TEAMS.
IF YOU WISH TO HAVE YOUR NAME REMOVED FROM THE SUB LIST PLEASE REACH OUT TO THE BOARD FOR ASSISTANCE.
