Is a team needing you to be a substitute? Follow these steps to register.

STEP 1: Fill out the following forms and provide them to the team captain.

STEP 2: Provide Team Captain/Representative

2. Team captains/representatives will submit these forms to the GPLVL Board prior to game start.

Get put on a contact list as an available substitute for teams who are looking for a temporary player. Please fill out the following to be put on the contact list. Please note your information will be shared with GPLVL registered teams in order to contact you about filling in as a substitute.
